Records 1-2-3, Inc. was launched in 1999 to provide paper templates for documenting patient visits in Family Practices. Dr. David Vinson developed the system first for his own practice beginning in 1995. By 1999 he had created a comprehensive set of templates for documenting over 40 scenarios including symptom based and diagnosis based visits, enabling providers to capture multiple complaints quickly and easily.

In 2010 Records 1-2-3, Inc., working with Direct Focus Software, LLC., introduced an EMR implementation of the same proven design known as AvivaEMR for Family Practice.

The system is affordable and scalable from a single laptop to 100's of users. It currently runs either standalone or in a client/server environment, but is being tested in a web-based configuration using secure Windows clients over the internet.
