More EHR Consolidation – Modernizing Medicine Acquires gMed

I’ve been writing and tracking the EHR consolidation world for a long time. It’s always fascinating to me to see how various EHR companies merge, shut down, or build their own little (and sometimes big) empire. Today I was greeted by the announcement that Modernizing Medicine had acquired gMed. I found the announcement pretty ironic since I keynoted both companies EHR user conference last year. So, I know both companies really well.

On face, they are both companies in southern Florida and so that’s going to make the transition a lot easier. Not to mention the competition for talent in the area as they grow will be a lot easier with the combined company. I talked to Dan Cane, CEO and co-founder of Modernizing Medicine, and Joe Rubinsztain, MD, CEO and founder of gMed, today about the acquisition. They told me that the combined companies will have roughly 10,000 specialty providers and 420 employees. Plus, Dan Cane commented that they expect to add over 50 to that head count over the next little while. Dan also noted that if you use meaningful use attestation data (which is a decent, but faulty proxy for EHR market share) then it puts the combined companies in the top 15 of EHR vendors. That’s pretty interesting considering they only serve a handful of medical specialties.

I’ve recently written about the power of a specialty EHR company and both of these companies had been executing the specialty specific EHR approach with great success. Who doesn’t like a piece of software that’s customized uniquely for their needs?

I asked Dan and Joe about the future plans for the two software platforms. They’re cognizant that gMed has been so successful in the marketplace because of the tight integration between its PM, EHR, and endoscopy report writer. So, I got the feeling that Modernizing Medicine will be very careful to not “disrupt the apple cart.” Although, I think Modernizing Medicine is keenly interested in taking gMed’s 18 years of experience in gastroenterology and incorporating that knowledge and expertise into the Modernizing Medicine technology. So, I think we’ll see this play out slowly and once Modernizing Medicine has caught their product up to gMed, we’ll see them sunset the gMed software. That could be a year or two at least.

There’s one caveat to all of that though. First, gMed has a PM and Modernizing Medicine doesn’t have one. I asked if Modernizing Medicine would start using the gMed PM. They won’t (at least for now) and I think that’s because Modernizing Medicine wants a truly integrated PM if they’re going to roll out their own PM. Plus, gMed’s PM has so many gastro specific features that I’m not sure it would be a good fit for a dermatologist (one of Modernizing Medicine’s best specialties). My guess is that Modernizing Medicine will utilize gMed’s PM experience to finally roll out their own PM, but we’ll see. They certainly have plenty to tackle when it comes to gastro EHR and deciding what they want to do with the endoscopy report writer which is very popular and important part of what set gMed apart from other EHR vendors.

I respect both gMed and Modernizing Medicine. So, I’m really interested to see what’s going to happen with the combined companies.

The Value of an Integrated Specialty EHR Approach

As many of you know, I’ve long been an advocate for the specialty specific EHR. There are just tremendous advantages in having an EHR that’s focused only on your specialty. Then, you don’t get things like child growth charts cluttering your EHR when you don’t see any children. Or taken the other way, you have child growth charts that are designed specifically for a pediatrician. This can be applied across pretty much every industry.

The reason that many organizations don’t go with a specialty specific EHR is usually because they’re a large multi specialty organization. These organizations don’t want to have 30 different EHR vendors that they have to support. Therefore, in their RFP they basically exclude specialty specific EHR vendors from their EHR selection process.

I understand from an IT support perspective and EHR implementation perspective how having 30 different EHR implementation would be a major challenge. However, it’s also a challenge to try and get one EHR vendor to work for 30+ specialties as well. Plus, the long term consequence is physician and other EHR user dissatisfaction using an EHR that wasn’t designed for their specialty. The real decision these organizations are making is whether they want to put the burden on the IT staff (ie. supporting multiple EHRs) or whether they want to put the burden on the doctors (ie. using an EHR that doesn’t meet their needs). In large organizations, it seems that they’re making the decision to put the burden on the doctors as opposed to the IT staff. Although, I don’t think many organizations realize that this is the choice they’re making.

Specialty EHR vendor, gMed, recenlty put out a whitepaper which does an analysis and a kind of case study on the differences between a integrated GI practice and a non-integrated GI practice. In this case, they’re talking about an EHR that’s integrated with an ambulatory surgery center and one that’s not. That’s a big deal for a specialty like GI. I’ve been seeing more and more doctors starting to talk about their displeasure with their EHR. I think much of that displeasure comes thanks to meaningful use and reimbursement requirements, but I also think that many are suffering under an EHR that really doesn’t understand their specialty. From my experience those EHR vendors that claim to support every specialty, that usually consists of one support rep for that specialty and a few months programming sprint to try and provide something special for that specialty. That’s very different than a whole team of developers and every customer support person at the company devoted to a specialty.

I’m not saying that an EHR can’t do more than one specialty, but doing 5 somewhat related specialties is still very different than trying to do the 40+ medical specialties with one interface. One challenge with the best of breed approach is that there are some specialties which don’t have an EHR that’s focused just on them. In that case, you may have to use the every specialty EHR.

What’s clear to me is that most large multi specialty organizations are choosing the all-in-one EHR systems in their offices. I wonder if force feeding an EHR into a specialty where it doesn’t fit is going to eventually lead to a physician revolt back to specialty specific EHRs. Physician dissatisfaction, liability issues, and improved interoperability could make the best of breed approach much more attractive to even the large organizations. Even if it means they back into a best of breed approach after trying the one-size-fits all approach to EHR.

I’ll be interested to watch this dynamic playing out. Plus, you have the specialty doctors coming together in mega groups in order to combat against this as well. What do you think is going to happen with specialty EHR? Should organizations be doing a best of breed approach or the one-size-fits all EHR? What are the consequences (good and bad) of either direction?

I’m excited to tell you that we’ve officially announced the 2nd Annual Health IT Marketing and PR Conference happening May 7-8, 2015 in Las Vegas. My EHR User Conference Keynote – Where is EHR and Healthcare IT Headed?

Last month I was invited to give the keynote address at the gMed EHR User Conference. For those not familiar with gMed, it’s a Gastro focused EHR company out of Florida. Along with giving the keynote address, I loved the chance to mix and mingle with doctors, practice managers, and gMed’s staff to learn from them. No doubt, their experiences and insights will inform my future posts.

2000+ EMR and HIPAA Blog Posts and 9.5 Million Pageviews Later

We just passed 2000 blog posts on EMR and HIPAA (officially this is blog post #2009) and we’re right at 9.5 million pageviews since we first started this blog. Plus, to round out the numbers, we just passed 12,000 (officially 12,065) comments on the blog as well. I’m honestly not really sure how to process these numbers. Not to mention that nearly 5000 email subscribers get their “daily cup of EMR and HIPAA” in their inbox 6-7 days a week.

Over the years we’ve had a number of writers contribute amazing content to the site, but most of our regular writers contribute to another Healthcare Scene website like EMR and EHR. I’ve generally reserved EMR and HIPAA for the content that I write and so 1705 of the blog posts were written by me. Not too bad for a little over 8 years of writing. I think I’ve accomplished some good.

A scanner is one of those essential EHR peripherals and that won’t likely change for a long time to come.

gMed – Advertising since 8/2013 – If you’re in gastroenterology, then you need to take a minute to check out gMed. Great EMR and HIPAA Supporters

We have a number of really big announcements we’re going to make in the next month. One of them I can’t really mention, but the other is a Healthcare Marketing and PR Focused conference in Las Vegas (of course, since I live in Vegas). I’ve floated this conference idea to a couple healthcare marketers and their response has been nothing short of amazing. I think they’re really excited to go to a conference which helps them do their jobs versus going to conferences to market their company. Watch for the official announcement to come out soon. Plus, the other announcement I can’t talk about is even more exciting for the network.

With that said, Healthcare Scene (which EMR and HIPAA is a part) is doing really well. We’re well over 12 million pageviews and over 6500 blog posts since we started blogging on EMR back in 2005. One EMR Company’s View of CCHIT Certification

I posted the 2008 CCHIT List of Ambulatory EHRs and asked if any EMR Companies were interested in commenting on the CCHIT process. Joe Rubinsztain of gMed was willing to do a guest post on gMed’s experience getting the 2007 CCHIT Certification and the motivation for an EMR to become CCHIT certified. Joe provides some interesting insight into an EMR company’s perspective on CCHIT.

CCHIT certification is the government’s way to ensure a common EMR denominator. In principle, this is a good idea to help confused physicians choose interoperable products with common features. In practice, however, CCHIT testing is young and requires fine-tuning to ensure that all requirements are practical and relevant.

gMed recently earned CCHIT 2007 certification and we found the process useful, fair and expensive. The process did not help us design an easier product for Gastroenterologists, Urologists and Cardiologists, but it did help our customers understand our commitment to evolving standards. For 2008, CCHIT is placing more emphasis on interoperability, which may be cost-prohibitive for smaller or unsophisticated vendors, hence the lower number of certified products. We expect CCHIT to continue raising the bar, and we are committed to meeting new standards as long as they bring value to our users.
