I recently came across this graphic which tries to illustrate the components of population health management. I like the components they’ve listed around the outside, but I thought the center section was the most interesting. All of us in healthcare need to think about how we take population health management and incorporate it into a shared vision of what we want to accomplish. One thing is certain to me, population health management isn’t going to happen if we all stay in our silos.

The high level model and its components look correct. I feel the execution risk is heavily biased towards the engagement of the patient. This is a historical weak spot and given multiple chronic condition patients at the core of the problem, relying on the patient is a losing strategy. I believe the best approach is to leverage the health social network #healthSocialNetwork that surrounds the patient. That is the approach CareCliques and one that I feel will be proven to be a critical differntiator.
