Overwhelming List of EMR Companies
I’d been looking for some lists of EMR companies that I could use for my list of EMR and EHR vendors. I should expect an enourmous list since I’ve talked about how many EMR companies there are out there, but it is still overwhelming looking at some lists I’ve found. Good thing I’m young and will have time to go through it. I plan on going throuh and demoing many of the different EMR’s and will add them to my EHR and EMR Vendor list as I have time to do an appropriate EMR review.

ComChart EMR: An experience from the exam room
I was in the exam room talking to a patient, who I am treating for hyperthyroidism. The patient complained of profound fatigue, of several months duration. It was unclear if the problem was being addressed by the PCP. While talking to the patient, I reviewed all his labs which were in ComChart EMR. There was nothing obviously wrong.
From within the exam room, and while still talking to the patient, I connected to my hospital’s computer and download all his labs which were less than 1 year old. I then had ComChart file the labs into his ComChart chart. I then had ComChart EMR create a chart of all his CBCs by clicking the “Chart… CBC” button in the labs. It was obvious that his hematocrit had dropped precipitously sometime between January and March.
The PC computer I was using had Skype installed (a free program which allows you to make free telephone calls from your computer.) I also have a headset attached to this computer, which I use with Dragon NaturalSpeech Medical. While still in the exam room, and from within ComChart EMR, I clicked the “PCP” button in the patient’s Progress Note and a dialog box popped up and asked “Do you want to go to the Dr. XXX’s Addresses file?” I selected “yes.” I then clicked on the label “private,” which is in front of the physician’s “private” phone number. ComChart opened Skype and connected the call. I spoke with the PCP and I arranged for the patient to see the PCP the following day.
I then click the “orders” button and selected my “anemia work-up” panel. This created a lab order slip which included all the necessary blood tests. I choose the option “Send copy of results to… PCP” and then clicked the button “Fax order slip” to lab.
Finally, in front of the patient, I dictated a Progress Note, using Dragon NaturalSpeech 8 Medical. The resultant Progress Note included the chart of the patient’s CBCs. I then selected the PCP as the recipient, and clicked the button “Create queued fax.” In my office, I have one computer which continually sends out the “queued faxes” as soon as they are created. Thus, the PCP had received a copy of the Progress Note and the lab had received the lab order slip even before the patient left my exam room. The entire process occurred within a few minutes.
Needless to say, that patient was impressed that my office was able to use technology in order to efficiently advance their healthcare. And, the fact that it all happened in front of the patient, seem to reassure the patient and de-mistify the healthcare delivery process.
True story. It occurred on 7/7/06. Score one for ComChart EMR.

Quantity vs. Quality
There is another source you might consider to post to your site as it relates to presenting a Who’s Who list of EMR/EHR vendors competing in this space.
I’ve been tracking such players for quite some time now and while having a LARGE list of the rank `n file I suspect most visitors on your site would want to distinguish the value players from the wannabee’s?
If you request, I will provide you with a pointer or two. If you use ACT! the info would be even more useful to you and your readers.

Why is it so impossible to make everything?

I have thought about creating a list of some of the best EMR’s. This is obviously not the point of this list of EMR companies. However, it is a hard thing to decide which ones are the best and should be on a list too. That’s the way to piss off a bunch of EMR companies. I would suggest that most of this list of EMR companies has someone using their product and have it working.
We’ll see how much time I have in the future to work on a list of that sort.

Dr Zwerling’s comment above made me think of my old school approach. (I’m am old school doc):
I look at the patient. They complain of being tired and look pale. I pull down their eyelids to make my “guess the crit” estimation of anemia, ask my assistant to get their doc on the phone, and a few minutwes later I tell him to fit her in ASAP. No computer. Your first problem is that you think that you must make a huge expenditure in order to go electronic. You definitely do not. Sure, it does cost something. However, there are a number of EMRs that are quite cheap. Of course, if you’re an old doc, then maybe it’s not worth your time to learn the new technology. I don’t see anything wrong with that.
I just wanted to also mention that you don’t have to go as technical as Dr Zwerling did. His is a great scenario of using technology, but just because you use an EMR to store your records doesn’t mean that you can’t just call a doc on the phone as opposed to skype.
This site isn’t about convincing people to do EMR or not. It’s all about helping people understand EMR so they can make the best decision for their practice. Each practice is unique and so I hope to be able to provide information that will help improve successful EMR implementations and avoid the failures that people are experience trying to implement an EMR.
The “Logician” product you have listed has not been called by that name for about 5-6 years. GE changed the name to “Centricity EMR” shortly after they acquired MedicaLogic.

That’s not surprising. This list is really old and was posted a long time ago. The key point about this list is just showing that there are a LOT of EMR companies out there.
I’m currently working on listing the EMR companies in my EMR and EHR matrix of companies. So, hopefully over time with the communities help that list will be more complete than this static list above.
I’ve been in the medical billing field for almost two years and the company I work for just implemented and went live on the Medievolve system on February 16th. As of March 1st, we started receiving payments from major carriers such as United Healthcare and Medicare. I worked at the billing office and the program was mainly used in the individual physician offices and I guess because No one really knew what to expect, there has been a lot of complaints about the program. I know EHR is the here and now and I just pray that the bumps we are experiencing now will smooth themselves out very soon. Thank you for giving this information. Do you know of any systems that are comatable with home medical billers?

Sounds like you’re experiencing a common EHR implementation experience. Unfortunately, far too many implementations don’t manage expectations very well.
Assuming you have decent leadership, things should smooth themselves out over time and you’ll nearly forget the initial challenge of implementing.
As far as home medical billers, I’d say that every EMR could be done that way. Those that are web based are easier to implement home medical billing. However, I can’t think of any EHR that couldn’t be done from home using some sort of remote desktop or Citrix (which is essentially remote desktop on steroids) environment. I’d say this is one of the benefits of EHR.
Question: If and when we get a national EMR system that allows patient records to be passed from one facility to another, will all of these EMR products listed be compatible with the national system? Will there be a need for interfacing between some or all private systems and the national EMR “highway?”

Absolutely not. At least not in the foreseeable future.
I think you’re mixing terms a little bit also. Compatible and interfacing with the “national EMR highway” are essentially the same thing. If an EMR is compatible with it, then it will be able to interface with it. There might be various types of interfaces. Some more complete. Some real time. Some batch posted. etc etc etc. However, even with those options I’m quite sure we can be certain that many of the above EMR companies won’t be able to interface with the national system.
What will happen with those companies that don’t will be interesting to see. However, that’s still a LONG ways before it will matter.

There are a number of companies that are trying to use Vista (the VA EMR system) outside of the VA. They seem to be getting some traction. Although, I personally don’t know anyone that’s using this system in ambulatory care. I only have seen it implemented in a hospital setting.
I’d be interested to know of someone using it in ambulatory care if such an install exists.

The list of emr companies is growing, but they re not certified yet with the new certification standards. These standards have not been established yet. In addition, Meaningful use standards have to be established and that will impact the type of software a doctor chooses. Preactice Management Software is going to be an integral part of the package as well. Definitely true that NONE of the EMRs listed above can be considered “certified EHR” in order to get EMR stimulus money, because NO EMR is certified yet. Same with meaningful use. Still LOTS of information about both that still needs to be given.

You might also want to list Practice Fusion at http://www.practicefusion.com . They offer a web based system that is FREE to use. Financially backed (in part) by Salesforce.com, they also guarantee support for the HITECH standard and ability for physcian-practice to earn incentive payments.

This post was done in early 2006. So, certainly there’s a bunch of EMR vendors missing now. In fact, it’s amazing how much has changed since 2006. Although, this list has actually gotten larger instead of smaller.

You should look at Medscribbler as it is not on your lists. It is tablet based with multiple input options when charting clinically. Complete mobility for a provider or practice.

Depends on how you count them. I personally like the 300 EHR company numbers. If you add in other things like ePrescribing it’s probably more like 600. This list is definitely old. It’s the list as of 2006. So many on this list aren’t here any more and so many new ones are here.
The price and demo buttons on the top are updated. I could add more, but just haven’t had the time.

There is a new EMR that is rapidly taking over from current EMR systems and it is not on your list. It is predicted that it will be #1 in the US within 2 to 3 yrs? The company is One TouchEMR and it was created by a Doctor named Robert Abbate in Texas.
Why is it not on your list?

I think so. Unless you decide to take a drastically different approach and find doctors looking to find a new method for practicing medicine. Then their might be some opportunities, but that’s a much different sale than an EHR vendor as you might think of it.
It’s my understanding that a majority of EMR vendors have focused on the PCP community in their core product design. Many of these attempt to accommodate the specialty community (Cardio, Derm, OB/GYN, Psych, etc., etc.) with add-on modules to their core PCP product. Some vendors have focused on the needs of only a few clinical specialties (eg. OB/GYN only, or Derm only). Where would a list exist of EMR vendors that market to/service the non-PCP clinical specialties?

Your analysis is pretty accurate. Although, unfortunately I don’t think a list like you describe exists. Most EHR vendors are reluctant to release which specialties are best suited for their EHR. You might check with the medical society for that specialty to know more.
